Peter had it right. A file needed to be uploaded first. It's counter intuitive because the box at the top of avatar management says "Don't use an Avatar" so I frantically looked around for a button that said "Use Avatar"

OK you bright guys have this figured out, not me! I also have the radio button (that cannot be undone) that says don't use an Avatar. I do not see an upload option...

Bottom of that same page there will be an upload from file or url option.
